Reflect on a failed decision and redo it

Tell us about a consequential decision you made that ended up being wrong. (a) Give the background: the goal, alternatives you weighed, constraints, people involved, and the information you had when deciding. (b) Rebuild the reasoning you used—trade-offs, risks, expected value—and explain where it broke down; point out blind spots and biases that affected you. (c) Describe what told you the decision was wrong, how you fixed it, how you communicated across teams, and the results you could measure. (d) If you could do it again, what particular process changes, safeguards, or coordination practices would you put in place to stop it from repeating? Which of those have you already adopted?
Overview: This question tests judgment, ownership, risk evaluation, stakeholder handling, product impact analysis, and reflection after failure—all skills that matter for data scientists.
